     +  Check and discuss blood pressures.

     +  Consult regarding medications, health issues, recent illnesses,

         hospitalization or medical care.

     +  Aid with referrals to other community resources concerning:

         depression, dementia, abuse, addictions, home health needs, exercise

         and fitness.

     +  Discuss any areas or topics for adult education programs you would

         like to see offered in our community.

     +  Obtain health-related information or brochures on any topic of interest

         to you.

     +  Seek out or help establish support groups within our community.

     +  Pray with you.

     +  Arrange home visits, nursing home visits, or hospital visits.

     +  Help with any health-related questions or issues.
